20 Mishaps That Might Have Started Accidental Nuclear War This is an interesting list of might have beens. I was aware of some of the incidents on this list where Nuclear war was bareley averted but not all of them. Some of the alerts are pretty close calls and show how error prone any system with a human in the loop is. Odd how many incidents are detailed with US forces and only a few with Russian forces. i wonder if that is because Soviet Archives have not been fully opened yet? I find it hard to believe that a system as fantastically inefficient as the Soviet one did not have at least as many close calls as the Americans, if not more.
