I saw this “photo of the day” over at Wikimedia Commons and thought it striking enough to post here.
October 23, 1895 ? Gare Montparnasse, Paris, France: a local train overruns a buffer stop due to Westinghouse air brake failure and crosses more than 30 metres of concourse before plummeting through a window. One person killed in the shop below.

The Station must have rested on a hill or incline so that the trains were stationed on the second story. I'm sure that after this they second thought what kinds of safeguards needed to be in place to prevent trains from breaking through any barricades.
Very dramatic photo. Unfortunatly it seems things like this have to happen to get safer precautions in place.
